Hello humankindness Dignity Health Medical Group is the employed physician group of Dignity Health Arizona. Dignity Health Medical Group (DHMG) employs approximately 200 providers and 500 support staff that cover a wide variety of specialties. The medical group has had tremendous success over the past few years and now provides more than 73 subspecialty services. The physicians provide clinical services in their areas of specialty and many serve in pivotal academic research and leadership roles.DHMG is also heavily involved in preparing tomorrows healthcare providers. Responsibilities

SUMMARY - Provides psychosocial assessments, discharge planning/coordination, community referrals, crisis intervention and supportive counseling. This position will have integral role in leading Multi-disciplinary Team based care model in the Dignity Health Medical Group. The Social Worker will plan, organize and implement social work services for patients including determining referrals for abuse and neglect of adults and children, assessing and coordinating services for psychiatric/ suicidal patients, and determining safe discharges for vulnerable patient populations. This position will assist in instructing and educating doctoral residents regarding community resources, mandated reporting, and providing additional education as needed.

Qualifications

MINIMUM

Masters Degree in Social Work from an accredited university

One year medical or hospital social work experience, including post-masters and/or supervised internship

Experience in psychosocial assessments and safety planning, with a wide variety of populations.

Efficient documentation and communication skills.

Knowledge of Social Work Code of Ethics, National Association of Social Work (NASW) Standards of Practice.

LMSW license from the AZ Board of Behavioral Health Examiners

PREFERRED

Two years post-masters experience: medical, hospital social work, or out-patient primary care setting experience

Knowledge of best practice in clinical social work and regulation standards within a healthcare setting

Preferred experience with Allscripts and Cerner preferred.

Pay Range

$33.60 - $48.73 /hour
