Beyond care inside the hospital, Dominican is dedicated to the health and wellness of the Santa Cruz community.

Community programs include a Mobile Wellness Clinic, a tattoo removal program, and support groups for patients and new mothers. In addition, and in accordance with our mission, Dominican Hospital supports programs for the underserved in our community, including children and adults, students, seniors, and the homeless population. Under the direction of the Rehabilitation Director, the PPS (Prospective Payment System)/Intake Coordinator is responsible for the screening of potential patients and the facilitation and coordination of appropriate and timely admissions to the Acute Rehabilitation Unit. In addition, the PPS/Intake Coordinator is responsible for marketing to outside organizations that could refer patients to the programs, and educating discharge planners in the community about the Acute Rehabilitation Program and the services provided, as well as the criteria for admission.

The PPS/Intake Coordinator coordinates the overall PPS and outcomes data and assumes responsibility for data collection, completion and transmission of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) in accordance with Centers for Medicare and Medicaid Services. Specifically, the PPS/Intake Coordinator oversees all aspects of data collection on the IRF-PAI, ensures the presence of supporting documentation, and provides staff education to ensure compliance with internal policy and external regulatory requirements, including applicable Commission on Accreditation of Rehabilitation Facilities (CARF) standards, CMS guidelines. In addition, the PPS/Intake Coordinator develops and interprets policies and procedures and acts as the primary resource for immediate problem solving in regards to FIM scoring. This position requires a positive working relationship with interdisciplinary teams.


Required Experience:

  • Two years clinical experience in nursing or rehabilitation

  • Experience in IRF PAI coordination (preferred)

Required Education:

  • Graduate of an accredited school of Nursing, Physical Therapy, Occupational Therapy, or Speech-Language Pathology Program

Required Special Skills:

  • Must have leadership ability and good communication and interpersonal skills

  • Ability to function within an interdisciplinary environment

  • Ability to read and speak English clearly and be understood

  • Microsoft and Excel proficient

Required Licensure:

  • Current and valid California Registered Nursing, Occupational Therapy, Physical Therapy, or Speech Language Pathology license (RN, LIC PT, LIC OT, LIC SLP)

  • Current BLS Certification (CT CPRBLS)
