About This Position

The Hospice Liaison promotes continuity of care from the hospital to home by identifying appropriate hospice patients and by serving as a liaison between the patient, the physician, the hospital staff and the hospice office.

Acts as a resource to hospital staff, physicians and patients regarding hospice services and community resources. Identifies insurance coverage. Interfaces with Home Care and Infusion division as indicated.

RN Selection Criteria

Two years professional experience with one year of home health or hospice agency or discharge planning experience and one year in an acute hospital setting. Graduate from an accredited school of nursing.

Two years professional experience with one year of home health or hospice agency or discharge planning experience and one year in an acute hospital setting. Demonstrates clear, effective written and oral English skills, and objective, active listening skills. Excellent interpersonal skills and the ability to work with others across all departments. Demonstrates ability to be self-motivated and function with minimal to moderate direction and supervision.Demonstrates ability to organize and prioritize work efficiently.Ability and willingness to actively participate in hospital and agency inservices, task forces and committees as related to home care. Ability to achieve results for the organization with and through people, utilizing creative and innovative solutions. Current California Nursing License, CPR certification, and California driver?s license with proof of liability insurance for automobile.
