Position Summary:

The Ortho/Spine Clinical Navigator Program Coordinator at St. Joseph's Medical Center (SJMC) is an individual with expertise in orthopedics, acts as clinician, care coordinator, educator, and patient advocate, and is responsible for the development, coordination and maintenance of the care continuum for orthopedic and spine patients. The Program Coordinator provides leadership for all aspects of the continuum of care beginning before surgery through 90 days post operatively. This includes: pre-admission testing, perioperative services, inpatient unit, and all other applicable entities within the orthopedic service line related to evidence-based practice for orthopedic/spine patients. Assists in the planning, development, and implementation of program goals and objectives. Coordinates and oversees the ongoing disease-specific certification process to continually improve care for the orthopedic and spine patient population. Works collaboratively with physicians, physician office staff, Hospital nursing staff, care management, home care programs, and rehabilitation facility staff to promote a successful program and exceptional patient experience. Assists with onboarding education of new inpatient nursing staff. Collaborates with multidisciplinary groups at SJMC to develop and provide patient education and coordinate care on an ongoing basis. Assists the patient and family in the smooth navigation of their needs throughout the ortho/spine surgery journey. Oversees the quality metrics for the program and works collaboratively with physician offices, pre-operative testing and admission, peri-operative and inpatient care areas to develop pathways/plans to address issues. Knowledgeable on joint and spine program certification requirements and responsible for initial joint and spine certifications and ongoing re-certification.



  • Current and active CA RN license

  • BSN (or in progress)

  • Five years Medical/Surgical/Orthopedic experience

  • Two years nursing management/leadership/education experience

  • Experience in physician relations and patient experience/satisfaction

  • Orthopedic Nurse Certification within one year

  • Current BLS certification

  • Flexible schedule/hours

  • Proficient in English.

  • Proficient with computerized systems to support data tracking, presentations and clinical documentation.


  • Experience in development of specialty healthcare program designation

  • Peri-operative experience
