Responsibilities

The Licensed Clinical Social Worker is responsible for performing social work assessments and interventions as needed for inpatients outpatients and emergency department patients. They are dedicated to patient and family centered care that values personal self-determination skilled at managing multiple priorities at once and engaging in creative compassionate and ethical problem-solving. As a member of a multidisciplinary health team they perform comprehensive assessments and develop treatment plans that integrate the medical social and resource issues that impact individual patients families and at risk populations. Licensed Clinical Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

The functions of the Licensed Clinical Social Worker includes crisis intervention patient /family intervention high risk screening brief counseling referring for financial or other identified resource needs arrange and facilitate family/patient representative meetings with the health care team as needed assist in post acute placement on complex discharges and engagement of appropriate agencies or community resources when high risk patients are identified.

Qualifications

Minimum Requirements:

Experience:

  • A minimum of two (2) years in a health care setting which provides for the following knowledge, skills and abilities.

Education:

  • Masters degree in Social Work.

  • MSW

Licensure:

  • LCSW license

  • Current BLS through the American Heart Association.

Ability to:

Demonstrate knowledge and skills relevant to the principles of growth and development over the life span of the patient. Interpret knowledge needed to identify each patients requirements relative to his or her age specific needs. Perform psychosocial assessments and formulate appropriate plans. Assess and report incidences of suspected child abuse, dependent adult/elder abuse and domestic violence. Communicate psychosocial needs of the patient and family to the interdisciplinary team. Develop and maintain a professional working relationship with peers and medical staff members. Develop and maintain a professional relationship with community health, welfare and social agencies. Develop and present one inservice per year. Participate in 1 QM/UR study/committee per year. Maintain patients medical records. Maintain confidentiality.

Knowledge of:

Principles and practices of clinical social work in the healthcare setting. Individual and group consultation process. Principles of grief and loss

counseling. Crisis intervention techniques. Patients rights, ethical considerations and cultural differences in dealing with thehomebound patient. Community health, welfare and social agencies. Chemical dependency and mental disorders.

Pay Range

$36.96 - $53.60 /hour
